Child Health Record

The Child Health Record is an important record for parents to use and keep for their child. It consists of:

An Information Guide, for providing parents and professionals with child health information. This can be used as a guide for discussing your child's progress when you visit health professionals with your child.
A booklet for keeping a record of the health and development of children from birth to five and a half years of age, including all immunisations and growth details.
